Based on a 4/10's toggle time (which is relatively fast), it would take about an hour to count to decimal 8,192 or several hours to count to 65535.

Usually the index finger is used, or index and middle perpendicular for two down, or
a parallel / horizontal index finger for two or three down. (although, for three down,
you could use index, middle and third finger perpendicular)
The index-thumb up-down is a tweak-twist movement.
